We are looking for a qualified Home Health CNA.

 

The CNA provides personalized care to clients in their homes under the supervision of a registered nurse or other healthcare professionals. They assist with daily activities, monitor patient health, and support clients with mobility, personal care, and companionship.

Key Responsibilities:

1. Personal Care Assistance
   – Help clients with daily living activities such as bathing, grooming, dressing, and toileting.
   – Assist with feeding and ensure clients are consuming meals as per dietary needs.
2.  Mobility and Transfers
   – Assist clients with moving around the home, transferring from beds to wheelchairs, and using mobility aids as needed.
   – Follow proper lifting techniques to prevent injury.
3. Vital Sign Monitoring
   – Take and record vital signs (e.g., blood pressure, temperature, pulse, and respiration).
   – Report any significant changes in patient conditions to the supervising nurse.
4. Medication Reminders
   – Prompt clients to take medications as directed, without administering them.
   – Monitor for adverse effects and report concerns to healthcare staff.
5. Household Assistance
   – Perform light housekeeping duties like laundry, changing linens, and cleaning up after meals.
   – Assist with errands, such as grocery shopping or picking up prescriptions.
6. Companionship
   – Provide emotional support and companionship to reduce patient isolation.
   – Engage in conversations and activities suited to client interests and abilities.
7. Documentation and Reporting
   – Maintain accurate records of patient care, activities, and any observations or concerns.
   – Communicate regularly with the supervising nurse or healthcare team.
   Working Conditions:

May involve travel to different client homes.
Requires flexibility to work on weekends or holidays as needed.
